This is a mount for inductive probe used in 3D Printer bed autoleveling. I use it with a Fotek PS-05N sensor on my Voxel OX 8" 3D Printer. It has an adjustment bolt for fine tuning the z-height of your probe. The movement is possible through the use of a flexure (or compliant mechanism) that allows the sensor to move along one axis relative to the mount. With the current settings it can travel 1.5mm up or down from center.
This part is designed to mount to the side of 2020 extrusion (OpenBuilds V-Slot) and extends past the end of this extrusion.
To customize one, the most important setting is the distanceToMount, which is the vertical distance from the sensor mounting holes to your chassis bolt position. If you can mesure this within ~1mm accuracy the mount can adjust the rest of the way. Make this measurement while nozzle is at bed level and sensor is just barely triggered.
The nut on the end of the adjustment bolt must be a lock nut(marked red in the assembly image), and is designed to spin with the bolt during adjustment. The washers on each end also facilitate this spinning. The captured nut on the flexure shuttle moves up or down as the adjustment bolt is turned.
Hardware required:

2 M3 x 25 bolt
2 M3 nuts

1 M3 x 30 bolt
1 M3 nut
1 M3 nylon lock nut
2 M3 washers
