Remixed to fit my entirely too specific PBSM with E3D hotend FULLY INSERTED. To achieve this I just cut a small notch into the top fin of the heatsink (also fan duct for the heatsink). Solid fit, completely seated in the extruder assembly. I greatly prefer this to the stark 2mm of insertion you get by taking up the space with a 3d printed insert...
Because of the shorter hotend however, I chose to move the heated bed on top of the mounting plates it's typically underneath. This eliminates the worry of any additions to the hotend colliding with the base of the printer - the build plate is pretty much flush with the top of the base now. That means the heat block can be oriented with the bulk to the rear and out of the way and best of all no more need for turbulent cutouts in fan shrouds. I also took this chance to add some locknuts for manual bed leveling.
I'd already replaced the proximity sensor with a microswitch on the back, but since there's so little room between the proximity sensor mount and the end of the nozzle, this was probably a necessary modification as well. Combining that with finally having a perfectly level bed, there's no longer any wasted time waiting for shoddy calibration at the beginning of every print. It's the best!
