This is meant for version CUH-10XXA of the PS4 console. I never made an adapter for the later models, still thinking it works better than having another wire mess when dealing with USB drives, and slightly quicker.
Of course you will need to supply your own 5v/12v source. They make brick adapters available on Amazon that have a molex connector, like this:

You will also need a SATA cable like this:

You will have to cut/strip power wires and break them out to a 4 pin molex connector to adapt to power adapter.

The notches will fit around the tangs on SATA alignment / HDD cady, just simply use a soldering iron to melt the tangs to hold the cable in place.
I know, Iv'e been dragging my feet on this. Too many other projects... and the work life balance ;)
