This uses the same dimensions, I just extruded the bottom part where the BL Touch sits to cover the entire part and rounded some of the corners so it would not need supports. I have not printed this so no guarantees, but if it works let me know. I have an Ender 3 and am printing one that will sit slightly higher.
UPDATED 10-8-2018: If you find that this is too low for your sensor, I have made another one which will raise the probe up by 2.002mm (not 3.004mm as originally noted - thanks Lemaru), you can find the other model here:
