Update: This is pretty spot on now I've updated the design to lock in the lens as much as that helps out. I don't know what else to add to it now.
I'm still learning how to make my own designs, so this isn't the exact fit I was looking for, but it works out for my needs. I wanted to use my moment lenses with the google fabric case since they're both very nice products, so this plate fits in the camera spot good enough for the lens to attach. It seems to work without any adhesives and just how snug it fits between the case and the phone.
