This thing is a mouth piece for any hookah/ shisha. You can insert a whistle from a kids toy, to create the typical turbocharger sound.
I used this whistles:http://www.henbrandt.co.uk/search/t08%20221
(i found them sold on ebay with keywords: "lip siren whistle")
Also, I cut a houshold rubber to insert it into the sealing gap.
