Make sure you keep the long side of the heatblock to the side. This set-up results in a build volume of 205 × 190 × 190, wich is larger than the original hotend set-up. You can use the mount for dual E3D hotends, Or in my case, use the side that isn't used for cooling the print.
I've used this X-carrier extender, and cut it shorter so it extends the carrier for about 5mm:http://www.thingiverse.com/thing:1341972
This prevents the hotend from bumping into the belt when homing the X-axis.
I've used:
4× screws3×16mm for the fan (bottom holes) and the front of the mount
4× screws 3×20mm for the fan (top holes) and the back part of the mount
4× self locking nut
2× nut for the fan connector
The original 3×60mm bolts for connecting the mount and extruder board
I printed the mount at 100% infill to prevent wobling while moving the printhead. 60% would probably be enough, but I just wanted to make sure.
