You will need two M3 x 10mm hex screws to replace the two in top front of the hotend shrowd. Other than that, it prints pretty straight forward, and able to mount the extruder motor, with the original mount, at an angle on the top of the hotend. You will also need an extension cable for the motor, or extend the cables as I did.
Remove the PC4 M10 from the hotend. Leave the PC4 M6 in the extruder side to go over the PTFE tube, and cut the tube so that the ring on the PC4 M6 sits in the hole on the hotend.
EDITED: Note that this should be printed flat, and not on the back as in the pic above. The supports will only be on the rear of the piece, and save a bit of filament.
