This is an anvil horn grip training tool, intended to train your pinch grip to be as strong as blacksmiths have.
To complete the tool thread a length of rope through the hole, tying a knot on top (you can use a washer to distribute the load), and making a loop on the bottom (e.g. Surgeon's loop knot).
Then attach a carabiner to the loop with some weights.
To use it, orient the tool so that the thicker part is at the bottom, making it slip out of your palm if not gripped tightly enough, grip the walls of the tool and lift up while keeping the tool vertical.
Make sure not to get your fingers over the edge to hold the bottom of the tool, as then it's no longer a grip exercise!
